Whether it is having a chat and a cup of tea, supporting you with hobbies and interests, reminiscing about the past, going for walks or getting some fresh air in the garden it is nicer to do this with someone who cares.

End of life care

Most people with a life limiting illness want to stay living in their own homes and our end of life care services are here to help you do that. Supporting you to enable you to be as comfortable as possible in your own environment for as long as possible.

Dementia & Alzheimer’s

We understand the difficulties associated with Dementia, for both those living with the condition and their loved ones. Our aim, is to ensure that we understand your individual circumstances so that, with the advice of all relevant experts, we are able to enhance the quality and enjoyment of life.

Each individual with Dementia is unique. Our approach to help is to fully understand you, by consulting with you, your loved ones, your GP and your Dementia consultant.

The care we provided is based on a support plan that has been put together by all involved people, to make sure it is well suited. As times situations change, we adapt the care plan to make sure the support we give is always suited.

Our professional approach to care means that we will not only establish the most appropriate Support Plan, but that we will also provide extensively trained Carer’s with a compassionate approach to delivering care. We believe many people are happier if they can remain independent in their own homes and we aim to provide support to enable this to happen, through compassionate planning and communication.

Hospital Discharge Support

We are all eager to get home after a hospital stay, however there may be additional difficulties which means you may need additional care to support the transition from hospital.

We can help to prepare your home, for your return, by doing practical tasks like getting in the basics, tidying up and switching on the heating. We can work with other agencies to make sure you have any equipment or medication, so all you have to do is come home and get well. We can help to put together a recovery plan, where the amount of help you receive reduces as you get stronger.
And much more

Amazing Homecare also provides Supported Living services to people
with Physical Disabilities or Learning Difficulties. This includes helping people go to work, attend a day centre, participate in leisure activities or visit the shops.

Respite care

If you are relying on a loved one or family member to support you, and you want to give them a break, we can step in with respite care. Respite care provides temporary relief for a primary caregiver, enabling them to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member. We can tailor your support, so regular routines are kept and there is as little disruption as possible.

Wellbeing and Preventative care

Our elderly people are often known to neglect themselves as things become difficult. Taking extra care of your loved one’s health as they age is very important and being alert to changes and taking preventive measures can help to keep our loved ones well. With ageing, there are certain risks and signs which, if ignored, may lead to ill health and sometimes hospitalisation. As we see you regularly, we can help to look out for changes and may notice things that you do not notice or ignore. We work with District Nurses, GP’s then help to book appointments, liaise with pharmacies to make sure you have the correct medication, on time.
